The Loyalty Rewards program is our way of thanking you for your loyalty!  

 

TERMS AND CONDITIONS:

1. When you join and become a member of Loyalty Rewards you will be awarded one loyalty point for each dollar spent at Midwest Family Health and Simply Sunflower Gifts.

2. For every 100 loyalty reward points you accumulate, you will receive a $5 certificate.

3. Reward certificates will be delivered through the US Postal Service or by email and must be redeemed by the expiration date noted on the certificate.

4. To earn points, you must present your Loyalty Rewards card each time you make a purchase. Points cannot be added after the fact.

5. Only one Loyalty Rewards card may be submitted for points per visit.

6. The Loyalty Rewards card is not a charge card.

7. The Loyalty Rewards card has no cash value.

8. There will be no cash refund and no points earned when redeeming Loyalty Rewards certificates.

9. Points will not be earned on prescriptions, photo center, tax or sale of gift cards.

10. Loyalty Rewards cards and certificates are nontransferable.

11. Lost/Stolen Loyalty Rewards certificates cannot be replaced.

12. Loyalty Rewards cannot be redeemed in combination with any other coupon or discount offer.

13. Earned points will be carried over annually. You never lose a point in Loyalty Rewards.

14. Store employees, and corporate and institutional accounts are not eligible for memberships.

15. Loyalty Rewards will not rent, sell, or provide your name, address, or any other personal information to any other company or organization.

16. Loyalty Rewards members must keep their personal information file up to date.

17. Midwest Family Health & Simply Sunflower Gifts, at its sole discretion, may cancel Loyalty Rewards at any time.
